Agile Transformation In Large Healthcare IT Programs

Abstract

Agile methodologies, developed and validated in software product development environments, have been increasingly applied to large healthcare IT programs—including enterprise care operations platform implementations, EHR deployments, and care management system transformations. However, the translation of Agile principles to complex, multi-stakeholder healthcare IT programs is non-trivial, and the field lacks a nuanced analysis of the specific adaptations required for Agile to deliver its promised benefits in this context. This article examines the conditions, adaptations, and tensions that characterize Agile transformation in large healthcare IT programs. Drawing on project management, organizational change, and healthcare informatics literature, the article argues that large-scale Agile adoption in healthcare IT requires reconciliation of Agile's iterative, team-autonomous orientation with the contractual, regulatory, and clinical governance constraints foundational to healthcare program delivery. The article examines the principal failure patterns of unadapted Agile adoption, presents a framework for scaled Agile implementation aligned with healthcare program characteristics, and addresses specific challenges in team structure, stakeholder engagement, compliance management, and value delivery measurement. The contribution is both analytical and prescriptive: it advances scholarly understanding of Agile in regulated, complex-system contexts and provides healthcare IT program leaders with a grounded framework for Agile transformation that respects the distinctive demands of healthcare delivery environments.
